Background: Metabolic syndrome (MetS) has emerged as one of the most critical global public health challenges of the 21st century, doubling the risk of cardiovascular events and significantly increasing all-cause mortality. Moreover, epidemiological data reflect a sharp, alarming upward trajectory in its prevalence. Pathophysiology: This review explores the synergistic pathophysiological network of MetS. Visceral adiposity acts as the primary cellular engine, driving systemic insulin resistance. In turn, these core abnormalities trigger a cascade of downstream components, including atherogenic dyslipidaemia (the "lipid triad"), neuroendocrine-driven arterial hypertension, and systemic pro-inflammatory and pro-thrombotic states. Diagnosis: We outline the evolution of diagnostic criteria. The consensus relies on abdominal obesity or elevated BMI as a basic criterion, combined with at least two additional factors (dysglycaemia, elevated non-HDL cholesterol, and high blood pressure). Therapeutic Strategies: Non-pharmacological lifestyle modification — specifically the Mediterranean and DASH diets, structured aerobic and resistance training, and proper sleep hygiene — remains the absolute cornerstone of management.
