Castration-resistant prostate cancer (CRPC) with bone metastases remains a significant clinical challenge, frequently presenting with severe pain, skeletal-related events (SREs), and compromised quality of life. Radium-223 dichloride (Xofigo®) represents an innovative alpha-emitting radiopharmaceutical targeting osseous metastases. This case report describes the clinical evolution of a 67-year-old male patient with CRPC and symptomatic bone metastases treated with Radium-223 at a nuclear medicine facility in Porto Velho, Brazil. The patient demonstrated progressively declining erythrocyte parameters, gradual elevation of serum creatinine, and significant reduction in alkaline phosphatase levels during treatment monitoring from November 2025 to May 2026. These findings were consistent with expected hematologic and metabolic responses to alpha-emitter therapy. The case illustrates the importance of continuous laboratory monitoring and the potential therapeutic benefit of Radium-223 in individualized pain management for CRPC patients with metastatic bone disease. Further investigation into the long-term efficacy and safety profile of this radiopharmaceutical in Brazilian nuclear medicine practice is warranted.
