Венозные тромбогеморрагические осложнения (ТГО) остаются одной из наиболее значимых причин периоперационной заболеваемости у пациентов с новообразованиями предстательной железы, особенно при наличии сопутствующей сердечно-сосудистой патологии. У данной категории больных риск ТГО обусловлен сочетанием онкоиндуцированной гиперкоагуляции, инвазивности хирургического вмешательства и хронических нарушений сосудистого гомеостаза, что формирует сложный клинический баланс между профилактикой тромбозов и риском кровотечений на фоне антитромботической терапии. Несмотря на широкое распространение трансуретральной резекции простаты и радикальной простатэктомии, в том числе лапароскопической и робот-ассистированной, специализированные алгоритмы стратификации риска ТГО с учетом урологического онкологического процесса и кардиоваскулярной коморбидности отсутствуют. Цель настоящего обзорного анализа предметного поля: систематизация современных данных о прогнозировании и профилактике венозных ТГО в периоперационном периоде у пациентов с доброкачественной гиперплазией и раком предстательной железы при сопутствующих сердечно-сосудистых заболеваниях. Материалы и методы. Поиск литературы проводился в соответствии с рекомендациями PRISMA-Scoping Reviews 2020 в международных и национальных базах данных за период 2015–2025 гг. В итоговый анализ включено 25 исследований, прошедших методологическую оценку по шкале Ньюкасла-Оттавы (Newcastle-Ottawa Scale, NOS). Результаты. Профиль ТГО определяется типом хирургического вмешательства, особенностями периоперационной антитромботической терапии и индивидуальными факторами риска пациента. Минимально инвазивные методики ассоциированы с меньшей кровопотерей при сопоставимо низком риске венозных тромбоэмболий. Полученные данные обосновывают необходимость перехода к персонализированным стратегиям профилактики ТГО и дальнейшего изучения интегральных методов оценки гемостаза и биомаркеров риска. Заключение. Минимально инвазивные методы радикальной простатэктомии сопровождаются меньшей кровопотерей при низком риске венозных тромбоэмболических осложнений, тогда как при траснуретральной резекции аденомы простаты особое значение имеют контроль гемостаза и рациональное ведение антитромботической терапии. Полученные данные подтверждают необходимость индивидуального подхода к тромбопрофилактике с учетом типа вмешательства и риска кровотечений. Introduction. In this population, THC risk arises from cancer-induced hypercoagulation, surgical invasiveness, and chronic vascular homeostasis disorders, creating a complex clinical balance between thrombosis prevention and bleeding risk during antithrombotic therapy. Despite the widespread use of transurethral prostate resection and radical prostatectomy, including laparoscopic and robot-assisted procedures, no specialized algorithms exist for stratifying venous THC risk based on the urological oncological process and cardiovascular comorbidity. Aim of this review of the subject field: to systematize current data on prediction and prevention of perioperative venous THCs in patients with benign hyperplasia and prostate cancer with concomitant cardiovascular diseases. Materials and Methods. We searched international and national databases (2015–2025) following PRISMA-Scoping Reviews 2020 guidelines. The final analysis included 25 assessed methodologically using the Newcastle-Ottawa Scale (NOS). Results. The THC profile depends on the surgical type, perioperative antithrombotic therapy, and individual risk factors. Minimally invasive techniques are associated with less blood loss and comparably low risk of venous thromboembolism.
