Diabetes is a chronic, progressive disease requiring durable, evidence-based therapies that address glycaemic control and broader cardiometabolic risk. Originator molecules play a key role in this landscape, supported by extensive clinical development, rigorous quality standards, established safety monitoring, and long-term real-world experience. Beyond efficacy, originator molecules provide confidence through consistent manufacturing, traceability, robust pharmacovigilance, and a well-characterized benefit–risk profile. These attributes are particularly important in long-term diabetes care, where treatment reliability, patient safety, adherence, and outcome consistency remain central to clinical decision-making. Semaglutide has emerged as a transformative therapy that extends benefits far beyond glucose control and weight reduction. Across the STEP and SELECT clinical programmes, semaglutide demonstrated substantial and durable weight loss, improvement in cardiometabolic risk factors, enhanced quality of life, and favourable effects on cardiovascular, kidney, and metabolic outcomes in people living with obesity and type 2 diabetes. Notably, semaglutide 2.4 mg reduced major adverse cardiovascular events by 20%, lowered all-cause mortality by 19%, decreased progression to diabetes by 73%, and reduced the risk of kidney disease or renal death by 22% in individuals with overweight or obesity and established cardiovascular disease. Additional evidence supports improvements in heart failure symptoms, physical functioning, obesity-related osteoarthritis, body composition, and liver health. By addressing the interconnected cardiovascular-kidney-metabolic disease continuum, semaglutide represents a comprehensive therapeutic strategy that delivers clinically meaningful benefits beyond weight loss, reinforcing its role as a cornerstone treatment for obesity and diabetes management. By integrating proven efficacy, quality assurance, safety evidence, and clinical confidence, originator molecules continue to support individualized, guideline-aligned diabetes management while contributing to improved outcomes beyond glucose control alone.
