Obesity is associated with chronic inflammation, metabolic disorders, and alterations in the secretion of myokines, cytokines, and neurotrophic factors. Myonectin, brain-derived neurotrophic factor (BDNF), and interleukin-6 (IL-6) are important markers associated with energy metabolism, neuronal function, and systemic inflammation that are impaired in obese individuals. The aim of the present study was to investigate the effect of eight weeks of high-intensity interval training (HIIT) and resveratrol supplementation, separately and in combination, on serum levels of myonectin, BDNF, and IL-6 in inactive middle-aged obese women. This quasi-experimental study was conducted with a pretest-posttest design and a control group. Forty inactive middle-aged obese women were randomly divided into four groups: exercise, supplement, exercise + supplement, and control. The HIIT program was implemented for eight weeks, three sessions per week, and the supplement group consumed 500 mg of resveratrol daily. Blood samples were collected before and 48 hours after the intervention, and serum levels of myonectin, BDNF, and IL-6 were measured by ELISA. Data were analyzed by analysis of covariance. The results showed that after eight weeks of intervention, serum levels of BDNF and myonectin increased significantly in the exercise and exercise + supplement groups, while IL-6 showed a significant decrease (P < 0.001). The most favorable changes were observed in the exercise group combined with resveratrol consumption, indicating a synergistic effect of the combined intervention. Combining HIIT training and resveratrol consumption can help promote metabolic health in inactive middle-aged obese women by improving neurometabolic markers, strengthening muscle-brain communication, and reducing systemic inflammation.
