The aim of this study was to analyze the current scientific evidence regarding the benefits and risks of pairing physical activity with metformin therapy for body weight reduction. The available evidence indicates that metformin exerts a modest effect on body weight reduction. Its beneficial effects may be attributed to improved insulin sensitivity, inhibition of hepatic gluconeogenesis, and modulation of appetite and satiety regulation. Regular physical activity plays a significant role in body weight reduction through improvements in energy balance and metabolic parameters. However, the available evidence is limited by the heterogeneity of the patient populations investigated across studies. Sources of heterogeneity included differences in age, degree of obesity, presence of metabolic comorbidities, and physical fitness, which complicated the interpretation and comparison of study findings. Further long-term studies are required to more conclusively establish the effects of metformin and physical activity in specific patient populations.
